Measurement Survey

Dear CSS Members:

Enclosed is a survey that I’d like all surgeons to complete. Our group is founded on the concept that “we are a group committed to recording the end result for every patient, determining the best practices and optimizing outcomes in shoulder surgery, just as Dr. Codman envisioned.” This is our mission as stated on our home page. To that end, I’d like to assess how we are living up to this commitment. This questionnaire will allow me to not only know what our commitment is, but it offers the opportunity for me to propose a business opportunity to all of you.
