How Long Should You Immobilize the Shoulder After an Arthroscopic Rotator Cuff Repair?

Enclosed is an interesting article you may have missed. It is an excellent example of how a Hypothesis-driven prospective study is so powerful. I bring you this from my former fellow Florian Grubhofer, who just sent it to me. The title is “Effect of Abduction Brace Wearing Compliance on the Results of Arthroscopic Rotator Cuff Repair.” While this study evaluates compliance with an abduction brace for six weeks following a small to medium rotator cuff tear, it also has implications for when you may want to start PT postop. In the past, when we tried to come to some consensus about aftercare for arthroscopic rotator cuff repair, it felt like we had opinion trump evidence, as everyone had a different opinion with worry about stiffness and the need for early PT. I think it is clear to me from this study, as well as others which consider risk of stiffness, that more extended immobilization is better if you really want an ideal rotator cuff repair to heal. Moreover, this is even more important for larger tears.
